Since you stated you are going to replace the carpet I recommend spraying "Odoban" (about $10 for a gallon to dilute) or some other cleaner/smell remover over the pad (if you don't replace it since it will hold the odor) or on the floor if you remove the pad.

I was going to do a SERIOUS OZONE treatment but then just opted to do duct cleaning and the complaining stopped after that.I'd make some attempts like duct cleaning to help and then say it is what it is...Also, has the tenant identified any specific area where the odor is coming from?

Well I look at the faucet and it needs to be replaced , but I tell the tenant that I will be back in 4 hours , clean the place , I cant work with the odor of garbage and a sink that is growing a science experiment .

@Erik Whiting  over the course of the decades working with clients another very easy way to determine the status of your renter is to look at their car.. if its a pig sti  so is the house 90% of the time I work out of my car a lot so its probably a little more messy than some.. but you know what I mean.. you can just tell.. and I really like  the ODOR  thought process you can smell folks that are un tidy they have a certain odor to their cloths.. as one who has worked in the distressed space for so many years .. you just know it..

I think the tenant's furniture, mattresses, clothing, etc. sucked up the odor or camouflaged it.Unless, you are desperate to rent the unit then find a better tenant with less risk.
