There is a distinction that NYC makes when it comes to "habitable" vs.

FHA appraisals can be hard to predict, but they're focused on safety/habitability and that is ultimately the opinion of the appraiser.

That said, I see no harm in disclosing your former license especially if you believe you may become licensed again.

There is a reason the units are vacant - most likely because the current owner knows he/she will not get the rents to cover the costs of making them habitable.   

Then go ahead and place that document (LOI and PoF) in an envelope and mail it to the PO Box in Pennsylvania, again dated the current date. 9 times in 10, this is where all their other bills go, so it's a mailing address they actually check.If the listing broker is doing their job, then there's no harm and no foul.
