The letter is from a valid doctor, one that happens to live 120 miles from them.

Just ask them to get an up-to-date letter from their doctor. 

You may have a Doctor's note but are under NO obligation to share it with anyone (which is also a HIPAA violation).

Also if this is a doctor with a temporarily depressed income before finishing residency/fellowship I wouldn't worry either.
