If you can legally require this as part of your application packet then failure to complete constitutes an incomplete application which is cause for denial of application.

When you file for eviction they ask if you have sent the 5 day notice or if it is in your lease.This is the notice:NOTICE TO TENANT: IF TENANT DOES NOT PAY RENT WITHIN FIVE DAYS OF THE DUE DATE, LANDLORD CAN START TO HAVE TENANT EVICTED AND MAY TERMINATE THE RENTAL AGREEMENT, AS THIS CONSTITUTES WRITTEN NOTICE IN CONSPICUOUS LANGUAGE IN THIS WRITTEN AGREEMENT OFLANDLORD'S INTENTION TO TERMINATE AND PROCEED WITH EVICTION.
