Maybe an error in construction, or maybe sawing tolerances way out of wack somewhere, or maybe saturated with water from bad flexible water pipe, or a leaking shark bite, etc..If it's water that caused it maybe it would be flexible when stepped on, maybe not.To me this does not seem like a cosmetic issue, and it might be a construction defect, or some kind of water damage.If it were mine, I would open the floor up and have a look.
