If the main stack doesn't run up the wall right behind the toilet (but be warned, they often do) You could probably slide the toilet down in the same (joist) bay, rotate it and put it next to the vanity (and have the tub, vanity, toilet layout) without compromising any joists and then just move the door to the bathroom over to where the back of the toilet is now.
