In general the larger a tile is the easier it is to get it to sit flat.

I believe the toilet sitting on top of it is ok as it can still move under it if needed.4- A nice clean caulk line at the edge of the tub will work fine and look just fine.5- I have not had to repair any sections yet, but am anticipating it to be a nightmare...I hope this helps.
