If you don't want the property any more a 1031 is probably your best bet, unless it's got a huge capital gains capture in which case you could just take your tax lumps and collect your funds. 

That name can’t be used in those states if already used so pick something a little off.  

Is there something I need to do to get my phone number and name disassociated with these properties or at least made private?

If you can identify property you can look up property on county website.If you know the owners name, Google can do wonders to get contact info.Why do you think property is underperformed?
