It is logical to me that the chances increase appreciably if you're using a note through a private party or so non-canned lender who farms the loan out to a large servicing company.

If the closing agent fails to transfer the title to the farm machinery you and the seller agreed to exchange when you bought their farm, or any personal property for that matter, the title insurance will not apply.
