HOI and plumbing coverage

HI! I am trying to understand what exactly gets covered with plumbing on regular HOI. Is there any insurance that covers plumbing that is inside the crawl space below the home if the inside of the home is not affected? Meaning, nothing is backing up into the home, but the crawlspace gets flooded due to broken sewage pipes in the crawlspace which needs cleanup and pipe replacement. 

I am also trying to understand what is covered by HOI when there are plumbing leaks in a home with a slab foundation (no crawl space) OR a home with an unfinished basement and/or slab plumbing.

Any input and information from owners or insurance professionals would be welcome!!
