Deed and Title Issues

I'm working on a wholesle deal and the seller(Thomas) has the Deed in his name but as I did title search the title is in someone elses name. Even though the property has been vacant for over 20 years the property was never sold to Thomas and the title is still in Brenda's name. What steps can I pass on to Brenda so that she can rectify this situation? Property is located in Philadelphia, PA
