4 Cap & Cash Flow

How can a 4 cap ever generate positive cash flow? Or positive cash on cash? I know buildings in NYC often trade for 4-5 caps. Assuming 25% down and 75% financed. 

Do these investors eat a negative cash flow and simply wait for appreciation?
