where can I find a written offer form?

Hello,

I hear some people give offers over the phone if they are talking to a seller who they think is not motivated enough for them to run out and see the property. Does anyone know where I can find a form to send? Do you email this or mail it? Do you favor offering over the phone more than putting something in writing?

do you have a technique for constructing the offer if you don't have MLS access? Sorry for all the questions I just paid like 800.00 for letters this month, and I want to make use of all of my leads.
