Moving Property to LLC - how do I change the newly signed lease agreement?

I just signed a lease with a tenant on my first rental property (a condo) and today my CPA advised that I move the ownership from me personally to an LLC. I used Zillow Rental Manager for the lease and it does not have an option to have an LLC for a landlord - just space for first/last name of landlord. What is the smoothest way to switch from me to my LLC on the lease agreement? If the tax records show my LLC as owner, do I even need to make the change on my lease agreement?
