Notice to Vacate / Cancellation of Month-to-Month Forms

Hi Bigger Pockets community!

Does anyone have a template for a "Notice to Vacate" form for a breach of the lease that they would be willing to share?

Additionally, if anyone has a form for a cancellation of a month-to-month lease that would be very helpful as well!

I am an investor in the state of MN, but not sure how different these forms might be from state to state.
