Adding resident to the lease

Good day everyone!

I have a bit dilemma.  Currently I have a lease for Single Residential house with three people on it. They applied through property management and signed the lease through them. I am no longer retain the property management services at this point. The tenants are in the 4th month of a yearly lease currently. Now they mentioned to me they want to bring additional resident because the house is marketed as 4 th bedroom. They never mentioned this to me in their application nor mention this through property management before the lease signed. They don't want the rent to be raised for this request.

So I am trying to accommodate the request but I will say to them this is will be one time only, if one of them move out, I don't want to add more resident as a replacement. 

The problem is I don't have access to the California Associate Real Estate forms. I would like to get the forms to be filled out ( I can type up the same agreement with the same verbiage/ languages from the current lease that I had. (between the tenants and property management). 

Please advise, I really appreciate it.
