Private equity contract for a purchase

I have several properties I’ve collected over about 10 years and am looking to purchase one more but have maxed out my conventional sources for loans. I have found private equity and have been asked to submit a proposal as the party with the equity has not loaned money in this fashion before. It’s only $150k loan and I plan to use one of my other properties as collateral. Can anyone point me in the direction of some template proposals for a deal such as this? Thank you in advance!
