How to make a 5 year real estate plan

I am trying to make a 5 year real estate plan for myself. Can anyone tell me how I should/ or give me tips and examples on how they made theirs? from reading the blog articles it is clear that you need a plan. I want to make mine, but struggling through some writer's block.
