Question on Recording a Quit Claim Deed for a Rental in Phoenix

I am preparing to rent out my previous home which I [/b]fully own (no mortgage). I've already created a [b]LLC but need some with getting it Recorded using a Quit Claim Deed.
In the Quit Claim Deed, there isn't an exemption code that applies for transfer of title to my LLC so am assuming I need to include the "[b]Affidavit of Property Value":
- Do I use a [/b]Sales price of 0?
- Do I use a [b]Down payment price of 0?
- [/b]What date do I use for Date of Sale??
Appreciate any guidence that you experienced Phoenix landlords/LLC owners can provide!! Thanks!!
