Unconditional Mechanic Lien Release Forms

Hey there BP community. 


I have come across two types of unconditional mechanic lien release forms. One with the contract / payment amount in the release form, and one WITHOUT the $ amount written into the form, and just with the project address. I was curious to know what experience others have had, and if you believe one type of form is better than the other.

I am working on collecting a large amount of release forms from contractors and it is easier if we can just input the project address, but please let me know your thoughts.
