House Information Sheet

HI All~

I am looking to create an information sheet about each of our rental homes so that if/when an appliance breaks, or a part is needed, I have all of the information needed.  For example, tenant calls to say water heater is not working.  I pull up my sheet and see what unit I have, make and model, serial number and phone number of the company that installed it.  I was wondering if anyone has a sheet that they have already created that they could share before I re-create the wheel?  Thank you in advance!
