Roommate Lease Agreement

I was wondering if anyone happens to have a template for a roommate lease agreement. 

I have a 3 Bedroom home and am planning to rent out 2 of the rooms. Any type of help or guidance would be great!

@Aaron Ta @Chrissy Kim @Kyle Fanene Hey everyone. I'm in Indiana and do some house hacking and multi-family property investing. I am also a licensed insurance broker. I linked a lease template I primarily use. I received this from a Landlord Protection Agency. Its not iron clad for every situation or specific to your state, but could be a solid starting point with some good protections. Make sure to check your state specifics and amend as necessary. Also remember to check with your insurance agent to confirm you're properly covered, some insurance carriers have exclusions for "house hacking." https://1drv.ms/w/s!AhfdFshugp...    Hope this helps!
