Difference between $100k a year and $1M per year

What is the difference between a person doing $100k a year in cash flow from buy and hold rental properties and the person doing $1 million a year? What strategies lead to $1M/yr and how would you start out to get there?

I'm 25 and have a few properties, but I want to start thinking bigger and reaching loftier goals. My original goal was 50 cash flowing properties in 6 years, but I realized recently I'm not aiming high enough. What would you recommend to reach that next level of passive income?
