Investor question on a loan refinance

As a relatively new active self employed real estate investor, I have purchased both sides of a town home about 5 years ago. The value has gone up and am interested in obtaining a loan against the equity. Can you tell me what more info a lender might want so I can put it all together in a nice package. I have included some pertinent info below. Thanks everyone
