FHA Titled held as joint tennancy w/survivorship

We filled out all title paperwork and turned in 1 week ago stating JOINTBTENNANTS/SURVIVORSHIP.
Yesterday the title company called my fiance at work to cime sign final loan/closing dicuments and asked if he ran it by the lender if My name could be put on the title / deed.....
he said he thought so but somehow ended up going and signing the title/loan docs without me and without me added to title.
I'm sooo confused & hurt and don't know what to do
if he is mortgage, i sont even javr a bank account (stay at home mom).
