Title Transfer from a Person to a Single Member LLC

Greetings, 

Looking for some insight on how to transfer a property in my name to a LLC I created. There is an existing mortgage on the property. I've read several different posts on the subject but it seems like there are multiple ways to do this.

Not sure if a quitclaim deed is the correct method. I'm also concerned with the Affidavit of Property Value. I'm under the impression that the transaction is exempt, but it is unclear as to which exemption code this transaction falls under.

Any help is much appreciated.
