My tenant has someone else living with them now

My existing tenant has a roommate now, what paper work should I have the roommate fill out. Due to my tenant being excellent I've decided I don't need to run a background check or credit check on the new person since she has clearly been making payments on her own. All I want is for that roommate to be in my books.

All adults in the residence should be screened. Background check, credit check, and income check should be a baseline for all applicants.
