Transferring of lease, is it possible to transfer my lessorship?

My sister and I own a property together and I plan to transfer my shared property ownership to her. I am in a fairly fresh lease agreement, going to 2 years. My question is, can my sister take over the lease agreement? What would be the process of transferring my lessorship to my sister?
