Previous Landlord Authorization Form

I am screening tenants and two previous landlords have asked that I send them a landlord authorization form or a rental reference authorization form. I'm not entirely clear exactly what the title is, but it was indicated to me it was a request for information about the tenant.

I searched online, but I didn't feel I found exactly the form they would be seeking. Does anyone have a template they suggest or a form they use regularly for this purpose?

@Alexa Potocki it is pretty simple. All you need is a statement that says "I authorize previous landlords to release information about my rental and payment history." Have them print and sign their name. Some landlords want this to avoid legal trouble for releasing personal information.
