Form to send to Apartment Management about Tenant

Hi,
I have a tenant looking to rent a place. They are currently in an apartment complex and I am working to get tenant history information. The apartment manager asked me to send over a 'form' that they will fill out.  Anyone have a form they use or questions I should make sure to ask?

When you send it you need to include proof that the tenant gave you permission to make inquiries.  That should be at the bottom of your rental application.
