Brand New Tenant Wants To Have Grandmother Move In

My new tenant, who just signed a lease, individually, notified me that her grandmother wants to move in with her. The lease is two days old. Originally, in the pre-screening interview, she mentioned grandma would probably be a tenant, but later decided to live elsewhere. They then decided to stick with the original plan. (I guess?) My response was that it would be alright, as long as grandma applied and met the criteria.  What is the best way to proceed? Do I void the original lease and sign a new lease with both parties, or do an add-roommate addendum to the original lease? Are there better options? Am I overlooking anything? I'm a new landlord and any advice would be much appreciated!
