notice/amendment/disclosure for Covid-19 for tenants/landlords

Has anyone worked on a notices/amendments/disclosures for Covid-19 for tenants and or landlords?

I am wondering if there is a boiler plate form tenants can use to notify landlords of their hardship and what their intention is.  And if there maybe one for landlords notifying tenants that they are still obligated to pay rent and if unable to do so they must notify landlord and make a plan. 

Anyone out there hear, seen or thought about this?
