Tenant minimum requirements form - Illinois

Hi everyone,

I’m a fairly new investor and just listed a single family home for rent in Illinois. I have a pretty significant number of inquiries and showings scheduled after just the first day on the market.

I am also a licensed real estate agent so I want to be sure I have a rock solid minimum requirements form for all of my tenants.

Wondering if anyone would share something you have used/borrowed etc.
