Recommended Form to Notify of Rent Increase - California?

I have decided to give 60 days notice of a rent increase to my tenant in Modesto California.  Can anyone suggest where I can find a good form for this that would be in compliance with California laws, etc? If there is only one adult who signed the lease is he the only one who needs to be named on the rent increase notice or does his girlfriend who lives there also need to be included in the notice?   The girlfriend's  name is on the lease as one of the people living there ( along with her two kids under 18) but the man is the only one who signed the lease.
