Addendum to lease - LLC

Good afternoon,

We just converted our first rental property to an LLC. The deed now has the LLC as the owner. I read that we should update the lease with our tenant so the agreement is with the LLC and not us individually.

Do others just create an addendum or a whole new lease?  We are right in the middle of a year lease.  If it is just an addendum, does anyone have samples they are willing to share?
