Minnesota eviction question

Hey Minnesota landlords. (or attorneys) Im working on an eviction for non payment and some other lease violations. I was going through the form HOU101 and have a question about section 3 regarding the part where you have to check A and B or C.  I don't have my address in the lease, I don't have it posted on the property either so I don't know what the correct answer is. I have had one other successful eviction and I was referencing that form and it looks like I had only checked box A on that one. I'm guessing the judge missed this or Is it not material all the time.

@Joseph Hammel  i’m not looking at the form but I’m assuming you’re referring to the requirement that you have a physical address for the tenants to contact you at. That state law you can’t simply just tell them how to contact you IE cell phone or  text.   You have to have a physical address that they can serve you if they wanted to sue you. It’s fortunate that you got away with it the first time but it’s not likely to overlook each time and if   your tenants hire an attorney the case could be thrown out.     My recommendation is to read the landlord tenant guide booklet for the state statute regarding this and get into compliance. If not for this eviction for the next one.
