Maintenance Forecast / Capital Expenditures

Curious if anybody has (or can point in the direction of) a sheet that is used to predict things like major repairs, times these repairs would need to be performed, cost estimates, and the like. Essentially, it would be something to give a sort of "maintenance forecast" for any given property. I have been working on my own, and have called it Capital Expenditures, but I am interested to know if anybody else has anything (tips, tricks, and the like welcome) for predicting future maintenance/repair/upgrade costs.
