HELP - Moved out but did not give 30 Day Notice

I let my landlord know the day I was moving out last month. On the day I was leaving, he had me sign an agreement termination that states that I acknowledge that I am named on the lease, but also that I wish to terminate my responsibility on the agreement and release any claim to the security deposit. I had forgotten what I had signed. 

I asked this my landlord what it means and he said that they plan to create a deposit summary, but cannot do so until the unit is vacated by all units because my previous roommates are living there so they cannot restore the room back to its previous state. Does this mean until original roommate moves out or until everyone gets out? He also says that I am still liable if any payments fall through between any roommates in the future. 

My roommate and I have our names on the lease and put in the original deposit amount a few years back. The new roommate we had for the previously did not put down a deposit, but the landlord said that he is now on the lease now too. I was told that the agreement is not I don't know what to believe.

I asked a lawyer and they told me that I need to simply write a 30 day notice and send it via Certified Mail for him to remove me from the lease. If so, what should I write in the notice since I have already left? Should I state that I moved out on x date and wish to be removed from the lease? I am not sure how this works and I do not want to mess up. I was not aware that I had to give a 30 day notice considering that the landlord had discussed with all of us knowing that I would be leaving last month. Would I have to wait another 30 days before the landlord gives me an agreement to sign? Would the landlord even accept it at this point?

Right now, I just want to get my name off the lease. I don't even care about the deposit anymore. I am scared that I've ruined my life because I did not do one thing correctly.
