Changing name on Retail Lease with Personal Guarantor

Good morning All,

I recently rented out a retail unit of mine in Calfironia and the original lease had the personal name of the tenant, as they were initially going to do business as a sole proprietorship.  However, now they have formed a corporation and would like to lease to be under the corporation name.  I do not mind doing this.

However, as this is the tenant's first brick and mortar business (all personal financials check out and have been vetted), I want to err on the side of caution and create an addendum to the lease with the name change to the Corporation, but also add the tenant as the Personal Guarantor in case things do not work out as planned.

Is my plan of action correct?  Also, does anyone perhaps have a template for a Lease Addendum and Personal Guarantor that they do not mind sharing?
