Need help filling out Notice to Pay

Hi everybody,

I'm filling out my 3 day notice in Colorado but I'm not sure how to fill this part:

The covenant/condition with which you are to comply is (check one or both, as applicable)

 The payment to the landlord in the sum of $______________ being past due rent and owed to the landlord from _____________, 20 _________, to _____________20__________.

If on the lease agreement said rent due on the 3rd then I should type : from 3rd to 7th ( the date I plan to serve to the tenant) and do I need to include the late fee up to the date i serve the notice or I just need to put the : sum of (the monthly rent)?
