When I fill out and file the JDF 104 form (Motion for Entry of Judgment), do I also fill out the Verification and Acknowledgment section? It seems to ask for the Defendants signature and date - but the tenant is not wanting to sign it.

Do I just leave that part blank, sign the plaintiff line and have it notarized before I file it with the court?

As for the JDF 107 form (Order for Entry of Judgment), do I just fill out the caption and the address in the 2nd checkbox section?

All the other forms seem pretty self explanatory. The only thing that wasn't included in the instructions was that the JDF 98 (Affidavit of Service) also needs to be filed with JDF 104 and 107 above. Luckily the court clerk let me know about this. Now I just need to make sure I fill out the latter 2 forms correctly.
