Problem Tenant's girlfriend moved in, called the police on the PM

So I have this tenant in a triplex that's been problematic. He's worked on "projects" outside and left wood scraps and trash outside, which the city cited for code violations.  We got him to clean that up and he did again later (another code violation).  He had a dog when he wasn't supposed to.  We got him to get the dog out. Then his girlfriend moved in without notice.... and brought her dog.  I think we got the dog out, but the girlfriend is still there. The property manager let him know that she needed to apply and be on the lease, but he hasn't done that. He pays on time, but his rent is below market (market has gone up quickly). 

I would like to get him out due to the violation, but don't want him to trash the place in the process. Although a worker that went in there described the condition of the place as "nasty".I at the least I want to get the girlfriend on the lease and get the rent up to market, but I don't want to be too heavy handed and cause them to trash the place.

She's already called the police on the Property Manager who was inspecting the outside of the building, and then she gave the PM a bad rating on the Better Business Bureau. So they are really pieces of work. They are $100 below market rent ($600).

I would just like to do this the right way, either get him out or get the rent up to market and girlfriend on the lease, without causing a lot of problems for myself. I don't need more drama in my life. :|

Ideas from anyone with experience with problem tenants are appreciated!
