30-day notice for landlords

Hello,

I am trying to find a correct 30-day notice to vacate form for Cook county. What are the key points that have to be on this form and what is optional? What is the right place to get it notarized?

I hope to hear from a landlord who has an experience submitting this notice without an attorney.
