Any suggestions on where to find/correct a lease agreement and rental application?

Check out BP forms, go to tools and then scroll down to forms you may find something there.

Office supply places offer generic ones and websites have some you can download. some free some for a fee. 

I would also check with your state and county and see if they have a housing division, in Minnesota we have the MMHA Minnesota Mulit Housing Association and they offer all forms and classes and info for landlords. The leases they have are honored by the court system and are well made up. You can add addendum's to a lease to cover Crime Free/ Drug Free, Lead, Pets, and other items specific to your property along with the Security Deposit/ Holding Fee agreement.

Same info for rental applications.. 

Or go to a local big apartment place act like you want to rent and want to take an application and lease home to go over before you submit it. some have those form on their websites you can download and would give you a basic format. 

OR ask you lawyer to draw one up.
