Tenant wants family to stay in unit for several months

I have a tenant that's been in the unit for over a year. It's a three bedroom and the occupants includes the mother with her son and daughter. Tenant is quiet and pays on time every month. The tenant just emailed me this morning that she has family that needs to stay with her for "a couple of months". The extra occupants will be one adult and two boys (11 and 15).

My lease agreement states that number of occupants is three unless written consent is provided by the landlord. I'm not against it if it's only going to be for a couple of months. Has anyone come across this situation? How did you handle it?  Does anyone have a template I can use for the written notice (I have Brandon Turner's Rental Property book with forms and didn't come across any that were applicable)?
