Nightmare tenants

So I'm reading all of these posts about 'nightmare tenants' and I'm wondering...are these all tenants that were not screened properly or substandard tenants that were allowed in because there was no better choice at the time? Or are these tenants that checked out, looked great on paper and had great history and decided to cause problems after moving in? Has anyone has a tenants that passed screening with flying colors and then didn't pay / caused problems?
