I have a tenant that has repeatedly refused to have her guest of three months leave the property. I served her with a dc100c form for notice to quit/lease termination and cited the guest. Is this the correct form to use? I had looked online and it was a little vague on which form to use for this situation. Thanks.

@Shaun C. 100c seems like the correct form. 100a is for nonpayment of rent and 100b is damage to the property.  Do you have a clause in your lease regarding other people staying at the house with your tenant?  100c could be used to state the tenant broke a term in the lease.  Good luck.
