Form to inform renters of check-out procedure

Does anyone have a form that they provide their renters that shows what they have to do when the lease ends and they are leaving (i.e. trash, floors, carpets, general cleaning, etc.) and the costs that will be charged against the security deposit if they don't complete these items?  The property should be left in the same condition that they received besides normal wear and tear.  Thanks so much!
