Sample Lease Addendum

My NYC rental is being shared by two tenants.  One of the tenants needs to move out mid-lease.  The other tenant has found a replacement.  The new tenant is requesting a lease with her name on it.  I would like to create an addendum to the current lease.  I am hoping somehow on this forum has some sample lease addendums they can share.
