Does anyone have a template of a pay or quit notice that you would be willing to share?

Also any other templates also that mind be handy for in the future?

This is what I use and have a Process Server serve...

NOTICE TO PAY RENT OR QUIT
TO _________________________________________________________________,
and all other tenants in possession of the premises described as:

PLEASE TAKE NOTICE that the rent is now due and payable on the above-
described premises which you currently hold and occupy. Your rental
account is delinquent in the amount itemized as follows:
